[mc4wp_form] doesn't display

Hello,
At the end of the App home page, there is a form to subscribe to the newsletter called [mc4wp_form]

The form doesn’t display, expect the name of the form : [mc4wp_form].

Where can I setup/find this form ?

Is it possible to add it in the revolution slider ?
