How to stop my titles from repeating themselves?

I’ve just noticed that I currently have some posts with repeated titles, like this:

Q&A – Vocular App – How Deep is Your Voice?Vocular App – How Deep is Your Voice?

How can I stop that repetition, so I just end up with a title that looks like this:

Q&A – Vocular App – How Deep is Your Voice?

I think it inherits this from the site title in General, which is ‘Vocular App – How Deep is Your Voice?’, but I really don’t know why it feels the need to repeat it. I think this has something to do with Yoast’s SEO plugin, but I’m not sure.

I cannot see any repeated titles. Is this resolved? Can you link to the specific post with this issue? Is this repetition in the breadcrumbs by any chance – the post title itself cannot repeat.
