contact form

I had the contact form looking just like the one on the demo page and then I looked a few days later and the rows for the subject, name and email are shorter. i can not figure out how to get them back to how they are on the demo page. Also when I try to put “fitness classes” into a page it will not show the image, and on the hover it will show about a half inch on the top. Any help would be great. Thanks
