uploaded images not appearing

Hi,

I am not sure what happened, all of a sudden whenever i upload an image, the image fails to appear. see attached image.

Please advise what is the cause.

I am unable to upload either. Some issue with the installation. Can you pls check your server error log to see the exact error? I disabled all plugins but no luck and hence it is more of a server issue. You can check once by switching to default theme – the same issue persists with the Twenty Twelve theme.
