site title being added by default at end of all title tags

every one of my pages
in the <title> tag
adds the word incredivid (title of site) to the end
like see here

http://incredivid.com/visual-world/
<title>A visual World | Reflections on Storytelling from Incredivid.comIncredivid</title>

and this happens in all pages,
where is this happening, i need to stop having the site title being added by default to all the title tags of all pages
