Is it possible to add fields to contact form

If I want to do a more complicated form than the standard Contact Form, is that possible using shortcode, or do I need to just code myself? For example, I want to move this one over:

I paste the code in, but when I do it makes the entire page go blank except the header on the new page:

Pls use the Contact Form 7 plugin for this purpose. We have made our themes compatible with that plugin and it is a very powerful plugin with loads of features and it’s free. Hope that helps. Thanks!
