Edit Donation Form

When setting up the form display options within Donations, is it possible to add more fields to the Registration Form Only option? If so, where do you modify? “Screen Shots Provided”

I am trying to use the donations functionality for an event registration and need more field that name and email. Does this require me possibly purchasing Give – Add On – Form Field Manager?

Any knowledge on how to achieve this would be greatly appreciated.
